SYNOPSIS:

On January 26, 1986 the Challenger Space Shuttle suffered a “Non-Survivable Incident”. The subsequent investigation concluded that a major causative factor was what has come to be known as “Normalization of Deviance”: Incremental seemingly minor changes that over time resulted in fatal flaws and a catastrophic loss!

Other losses I have investigated and read about emphasize lessons that must be learned to prevent other losses, initially “Normal” from becoming “Catastrophic”. There are also some losses yet to happen as a result of ignoring the lessons!
